Thank you for agreeing to participate in our Golf Course Pesticide Use Survey! This
survey is intended for golf courses located within the 50 United States only. This is a follow-up survey to the
golf course profile, water use and nutrient use surveys. We encourage you to complete this survey regardless of your participation in any of the prior surveys.

All of your individual responses will
be kept strictly confidential.
As a thank you for your participation and full-completion of the survey, you will be entered into our prize
drawing. The grand prize winner will receive a new flat screen television (valued at $2500). Seven additional winners
(one from each agronomic region in the U.S.) will receive a $250 gift card. GCSAA members who complete
the survey will be awarded 0.50 service points.
Non-GCSAA members will be offered a TAN membership. TAN stands for Technical Assistance Network. It is a one year,
complimentary GCSAA membership registered in the golf facility's name who in turn will appoint a representative from the
facility to utilize the membership. This individual will have access to many of the GCSAA resources needed to help that

Getting Started
If this is your FIRST-TIME accessing this Pesticide
Use Survey, simply register by entering your e-mail address and a password (of your choosing).
If you are RETURNING to the Pesticide Use Survey, please enter the e-mail address and password that
you used when you registered for the Pesticide Use Survey. This will enable you to update any of your
saved information.
NOTE: Participants in previous environmental profile surveys (such as the golf course profile survey
or water use survey) need to re-register for this Pesticide Use Survey.
